Very very irresponsible doctor to say the least.
My beloved father, only 48 years old, who was due for transplant due to liver cirrhosis was admitted to H. N. Reliance Hospital in the month of Oct. 2025 with septic shock due to Spontaneous Bacterial Peritonitis infection.
He responded well to the treatment and eventually came out of the septic shock & the SBP infection with normal WBC counts.
However, during the course of his stay in the hospital he developed a laceration on his scrotum which quickly turned into a big aggressive lesion.
All this while, his WBC counts were progressively rising again but all culture tests(ascitc fluid, blood, urine) were coming negative.
Finally, a uro reference call was raised on the 23rd of Oct.
This 'wise' doctor visited the patient on the 24th & suggested dressing with T-bact ointment & follow-up for the scrotal lesion.
The following morning of 25th Oct, the patient suddenly deteriorated, went into septic shock and was put on ventilator.
It was only then that the 'wise' doctor made the diagnosis of the scrotal lesion to be Fournier's gangrene.
In the absence of any other obvious source of infection, it was concluded by the primary liver team and this surgeon that Fournier's gangrene was the cause of septic shock for which this doctor suggested debridement and right orchidectomy-a highly risky procedure in a liver cirrhosis patient.
The rationale communicated to us was to reduce the infection burden by removing the necrotic tissue and also to identify the causative organism.
The surgery was performed on the 27th of October and went uneventfully thanks largely to a good anaesthetist.
However, this 'wise' doctor, in all his wisdom, for reasons best known to him, chose not to/forgot to collect a swab sample for culture sensitivity testing at the time of surgery and only sent specimens for histopathology.
All this while, my father being given antibiotics empirically.
Two days later when the primary team of doctors looked up for the culture report so as to initiate the appropriate antibiotics targetting the causative organism, to their utter shock, they were told that samples for culture sensitivity were never collected.
When the primary team arranged for collection of sample for culture sensitivity testing from the surgical drain on the 29th, this 'wise' doctor on rounds again downplayed its need. Only this time, upon the relatives' insistence, the sample was collected and sure enough, this culture test did throw up the causative organism & the effective antibiotics enabling the infectious disease team to initiate the appropriate antibiotics.
But alas, it was too late as the patient had become considerably vasoplegic during this uncalled for delay & sadly passed away.
This single negligent act of omission by the 'wise' doctor of choosing to not collect a swab sample for culture testing from an already open surgical site robbed my father of precious few days for the appropriate antibiotics to be initiated and act.
Especially when that was one of the justifications for the highly risky procedure violating one of the basic rationales of surgery - of diagnosing & detecting.
A few more seconds was all it would have taken to perform the crucial step of collecting a swab sample from an already open surgical site.
What was he thinking, that simply excising necrotic tissue will completely heal an active infection of the magnitude of Fournier's gangrene in an CLD patient and there is no need whatsoever for the physicians to know & target the causative organism with appropriate antibiotics.
As the saying goes, to a hammer, everything is a nail, to this surgeon, everything it seems is simply about on table surgical success.
The overall prognosis of the patient post surgery it seems matters precious little to him.
Just goes to show how some doctors can be 'intelligent idiots'. Would not recommend anyone to risk being under his care.
Can be incredibly irresponsible.

My husband was advised to undergo prostrate procedure for enlargement and had urine retention issue . We went to various doctors for advice but when we met Dr. Mangesh Patil at H. N.Reliance hospital, we decided to have the procedure performed by him.
Dr. Mangesh Patil was very calm , incredibly knowledgeable and listened to our issue and worries very patiently, explaining every detail about the issue and the procedure he would be performing on my husband. He is very approachable and pleasant doctor-patient interaction was one that I have not seen for years,as he made the whole issue so simple and easy. The procedure was successfully performed early July'25. He visited my husband at the hospital room, before and after the procedure daily and enquired with my husband which is an important factor to highlight. Most importantly, my husband's urine retention issue was resolved, which was indeed a big relief.
The hospital staff including the other doctors from Dr. Mangesh' team, nurses , PCA and ward boys were very helpful and prompt in their service.
I certainly 100% recommend Dr. Mangesh Patil to anyone who requires the services of an urologist at H.N.Reliance hospital and assure you that you will be in good hands always, keeping your worries and tensions away .Rest assured you can be at ease throughout the days - before, during and post procedure.

We are incredibly thankful to Dr. Mangesh Patil for the exceptional care and skill he showed during my father’s recent surgery. My father had a left renal mass that required timely intervention. Dr. Patil performed a left nephrectomy, removing the kidney along with the surrounding fat — and he did it with remarkable precision and confidence.

Post-surgery, he looked proud of the outcome and rightly so! When the biopsy results came in, it confirmed that the tumour had indeed spread to the surrounding fat. It was a huge relief to know that everything had been removed thoroughly and in time. All surgical margins came back clear, which was the best possible news for us.

The surgery itself was technically major, but thanks to Dr. Patil, my father’s recovery was swift and smooth. He’s doing well and is very happy with how everything was handled.
Couldn’t have asked for a better surgeon on our team during such a critical time.
